Therapy & Wellness Staff

Wyatt LaVigne, PT, DPT, ATC- Director TWC

Wyatt LaVigne is a Physical Therapist and Athletic Trainer, as well as the Director of the Therapy and Wellness Center. He works with a wide range of patients, but primarily with people with orthopedic or sports related injuries and athletes post-concussion. Wyatt also provides athletic training services to Falls High School and Rainy River Community College. Wyatt completed his Bachelor of Science in Athletic Training at St. Cloud State University and went on to complete his Doctorate Degree in Physical Therapy at the University of Minnesota in 2012.

Meghann Cassibo, OTR/L

Meghann Cassibo is an Occupational Therapist who has experience working with patients of all ages and in a variety of settings including hospitals, skilled nursing facilities, schools, out-patient rehabilitation, and home health with a passion for hand therapy and sensory integration. Meghann received her Master’s Degree in Occupational Therapy from The College of St. Scholastica in 2011. Meghann has been providing care at RLMC since 2013

Maria Clark, M.S., CF-SLP

Maria Clark is a Clinical Fellow Speech-Language Pathologist. She received her Bachelor’s degree in Speech and Hearing Science from Arizona State University and her Master’s degree in Communication Science Disorders from Baylor University. She has experience working with patients of all ages and in a variety of settings including hospitals, skilled nursing facilities, schools, and out-patient rehabilitation.  She treats patients who have speech and language difficulties, swallowing difficulties, and cognitive impairments.  Maria also provides services to Koochiching Health Services – Littlefork and Good Samaritan Society. She joined the RLMC team in 2022.

Heather Juen, PT

Heather Juen is a Physical Therapist who provides services to patients at RLMC, as well as, Littlefork Medical Center and the Littlefork Big Falls School District. Heather earned her Master’s Degree in Physical Therapy from the College of St. Scholastica in Duluth, MN. She has twelve years of experience in the physical therapy field. She began working at RLMC in 2016.

Krista Wagner, OTR/L- Occupational Therapist

Krista Wagner is an Occupational Therapist who has experience working with patients of all ages and in a variety of settings including hospitals, skilled nursing facilities, schools, out-patient rehabilitation, and home health. She received her Bachelor’s degree in Health Sciences and Master’s degree in Occupational Therapy at The College of St. Scholastica. Krista has been providing care at RLMC since 2018.

Hannah Junker, SLP-CCC

Hannah Junker is an ASHA-certified Speech-Language Pathologist. She treats patients of all ages who have speech and language difficulties, swallowing difficulties, and cognitive impairments. Hannah also provides services to Koochiching Health Services- Littlefork, Good Samaritan Society, and Lakewood Health- Baudette.

Brian Tolley, PT

Brian Tolley provides care to a wide range of patients, but primarily treats adult orthopedics, acute care and wound care. Brian has a special interest in chronic pain and has completed numerous continuing education courses on pain science. Brian received his Bachelor’s of Science Degree in Physical Therapy from East Carolina University in 1997. Brian has been providing care at RLMC since 1999.

Jennifer Baker, MSPT, CLT-UE

Jennifer Baker is a Physical Therapist. She primarily treats pediatric patients in the outpatient and school settings, but also sees a wide variety of adult patients. Jennifer is a certified upper extremity lymphedema therapist and has a special interest in women’s health services. Jennifer received her Master’s of Science in Physical Therapy from Des Moines University in 1997. Jennifer has been a P.T. at RLMC since 2003.

Kim Thorstad, COTA

Kim Thorstad is an Occupational Therapy Assistant who has experience working with patients of all ages and in a variety of settings including hospitals, skilled nursing facilities, schools, and home health.

Kim received her Associate of Applied Science in Occupational Therapy from Anoka Ramsey Community College in 1998. Kim worked as a COTA at RLMC from 2000 through 2009 and worked in the Twin Cities as a home health COTA for AEGIS and Home Health Care Inc. during 2013—2015.

Elizabeth Johnson, PT, DPT

Elizabeth Johnson is a physical therapist providing services at Rainy Lake Medical Center and The Good Samaritan Society-International Falls. She works with patients in the hospital, skilled nursing facility, and outpatient rehabilitation. She has a special interest in treating patients with vestibular disorders. Elizabeth received her Doctorate of Physical Therapy from Regis University. She started providing care at RLMC in 2019.
